We appreciate you reviewing the following information and completing the requested forms prior to your visit.  This will give our doctors a more comprehensive picture of your current health status and allow us more time for the treatment.

NEW PATIENTS:

• Please bring copies of your health related records to the initial visit, including medical records, specialty consult notes, imaging reports (Xray, MRI, CT scan, and Ultrasound), pathology results and lab reports.

• Please download and complete the following forms.  You may either bring them with you to the initial consultation or email it to info@acuprolo.com.
